PORTLAND, OR / ACCESSWIRE / June 21, 2024 / Comagine Health and Oregon Wellness Network (OWN) are pleased to announce the launch of the Advancing Health Equity to Address Diabetes (AHEAD) Community Health Concerns Survey. This effort is funded by the Centers for Disease Control and Prevention and aims to understand health concerns and perceptions related to diabetes among Oregonians, focusing on its impact on quality of life.

“We know the number of people with diabetes is on the rise and living with this chronic condition can be especially challenging,” said Evan Saurino, clinical director and family physician at Comagine Health. “Local residents who respond to this survey will help us identify and address important issues, ultimately improving access to diabetes care in communities across Oregon.”
The survey, which will run from June 15 to July 31, 2024, will provide partner organizations with the following benefits:
- Understand priority health concerns within your community
- Understand Oregonians’ perceptions of diabetes and how it impacts their lives and the lives of their loved ones.
- Survey results will be used to inform program recruitment and delivery and to support funding applications.
- Reach as many community members as possible in the target counties
Community members who complete the survey will have a chance to win a $25 gift card.
Comagine Health is a leader in community health work, working with partners to improve health equity across the country. Through their statewide partnership, Comagine Health and OWN are working to increase access to national diabetes prevention programs and diabetes self-management education and support services across Oregon. This survey will be a critical tool in expanding the reach of these programs.
